AMS Questionnaire: Low Testosterone Test

Is it low testosterone? This quiz can be used to track the progress of your testosterone treatment. When you are finished filling in the questionnaire, you will have the opportunity to save your scores and see them plotted on a graph over time. You also have an opportunity to print them in a format suitable for taking to your physician. This will help you discuss your low testosterone symptoms more accurately with your doctor.
