A Yellow Fall Risk Alert Program is an easy and effective way to promote patient safety in nursing homes and hospitals.
Purpose of a Yellow Fall Risk Alert Program:
- To establish a standard proactive approach to promote patient safety and avoid adverse events from falls.
Steps in setting up a Yellow Fall Risk Alert Program:
- Assess risk of falling and risk for a serious or major injury from
a fall. Perform standardized fall risk assessment for all patients
on admission and whenever patients' clinical status changes.
- Implement nursing interventions for patients at risk for falling.
- Communicate to all staff information regarding patients who are at risk
of falling and at risk of sustaining a fall-related injury.
- Provide patient with a fall
risk wristband.
- Place fall risk
magnet on patients bedroom door and/or on wall above their bed.
- Provide patient with fall
management socks/slippers.
- Provide patient with a fall
risk wristband.
- Monitor patients at fall risk. Perform hourly (or every 2 hours)
safety rounds to assess and address patient needs.
- Utilize Universal Fall Alarm
to assist with monitoring needs.
- Utilize gait belt to assist with safe patient
transfers and ambulation.
- Utilize hip protector for patients at
risk of injury.
- Utilize Universal Fall Alarm
to assist with monitoring needs.
