A Yellow Fall Risk Alert Program is an easy and effective way to promote patient safety in nursing homes and hospitals.

Purpose of a Yellow Fall Risk Alert Program:

  • To establish a standard proactive approach to promote patient safety and avoid adverse events from falls.

Steps in setting up a Yellow Fall Risk Alert Program:

  • Assess risk of falling and risk for a serious or major injury from a fall. Perform standardized fall risk assessment for all patients on admission and whenever patients' clinical status changes.

  • Implement nursing interventions for patients at risk for falling.

  • Communicate to all staff information regarding patients who are at risk of falling and at risk of sustaining a fall-related injury.

  • Monitor patients at fall risk. Perform hourly (or every 2 hours) safety rounds to assess and address patient needs.
