Understanding Bladder Conditions

Use this form to order your Understanding Bladder Conditions Post-Test and evaluation form for 5.0 Contact Hours and Certificate approved by the Society of Urologic Nurses and Associates, #23-09. You must have completed the independent educational activity available at http://www.verathon.com/ECManual.htm. Upon confirmation of payment, participants will be offered two methods of completing the exam. You may complete the post-test and evaluation form immediately, using an easy web-based form. Both forms must be completed to receive certificate. You are also offered the option of downloading a printable Acrobat document, filling out the test with pen on paper, then sending it by post or FAX to the examination supervisor. Corrected post-test will be mailed or FAXed back within six weeks. If you attain a passing score of 26 correct answers and complete the evaluation form you will receive a certificate stating the contact hours earned. If you don't score at least 26 correct answers, please review the program, available at http://www.verathon.com/ECManual.htm and take the post-test again.
